What can I expect?
About a week before each appointment, we’ll remind you to complete Patient Discovery and take a few minutes to share the topics you want to discuss with us in your appointment.
We’ll also ask you some questions about any challenges that may be impacting your health, like affording your treatment or getting to appointments.
Your responses will help us prepare for your visits and ensure you have the support you need throughout your care.
